Golden Prairie Manor seeks to encourage the admission of persons whose level of care needs, independent lifestyle, health conditions, and activities of daily living may be met through a supportive and individually personalized service plan. The approval of admission will be determined by the Administrator and the facility nurse, with collaboration from the resident's primary physician. An individualized care plan will be developed by the facility nurse with input from the resident and the resident's family. Additionally, resources such as other medical professionals, the family, and significant others will be utilized in the evaluation process. It is recognized that residents may no longer be adequately served by the programs and services of Golden Prairie Manor and will require discharge to a facility capable of meeting the needs of the resident. The determination of the need for discharge and the plan to discharge will be the responsibility of the Administrator and the facility nurse.

Residents of the Independent Living apartments must be mentally and physically capable of completing all activities of daily living (ADLs) with no daily assistance required from staff. Golden Prairie Manor is designed, equipped, and staffed to provide accommodations, care, and services for such persons who are mentally and physically capable of Independent Living. A resident may develop physical, mental, or social conditions requiring them to transfer to our assisted care division or another facility. Management reserves the right to make the final decision in determining the qualifications for residency at Golden Prairie Manor.
**In most cases, residents who transfer from independent living are able to remain in their current apartment. There are instances where we must locate them to a different area of the facility. These instances would be discussed with family prior to any decision of relocation for the resident.
